program! Simply fill out the Safe-D Pak Order Form and Payment Form, and we
will send you a prepaid UPS label; attach this label to your box, and the rest is up to us!

Safe-D Pak Service Overview
If you have Schedule 2-5 medications that you need disposed, our Safe-D
Pak is the perfect solution for you! We provide regulated disposal, inventory, issuance of DEA Forms
222 where necessary, and even disposal of up to 10 pounds of non-scheduled items. Simply complete and send in
copies of the Safe-D Pak Order Form and Payment Form, and we will return to you
all the materials you will need to send in your products.
